STEP BY STEP INSTRUCTION
- Step 1. Kid to Register a Source Code here:
- Q1. Key in your valid email. Without a valid email, no further step can be proceeded
- Q2. Key in Source code of the person who brings you here. If you don’t know, choose NONE
- Q3. Choose the list a Source Code for yourself. Remember this because It is your Source code to be used during the program, as an account name
- Q4. Choose Program P034 – Johari Space
- Q5. Click OK – join our News channel on Telegram for latest update
- Q6. Reconfirm your email address – must be valid & match with email on Q1
- By submitting the form, you will receive an auto email to your mail box, indicating all information you fill-in the form. Save the email in case you forget Email used to register, and/or Source code later
Note:
- Wait 40 minutes for the Source Code to be loaded into the program then proceed with next steps
- 1 email can register for 1 Source code only. Therefore, please use parent’s email to register parent’s account and kid’s email to register kid’s account, separately.
- Fill in the Authorisation form here between parent-children, so we can link report to both children & parent’s account later
Step 2. Kid to fill in Form A – about yourself here:
- Q1. Key in the same email you used to register Source code in Q1- Step 1, Kid’s valid email.
- Q2. Key in kid’s Source code (the one you chose on Q3 – Step 1. If you choose the wrong one, the response will not be updated & you will have to redo this step)

Step 3. Kids (parents should help) send Form B to your parents, teachers, friends, family members…, asking for their comments: What do they think about you?:

Note:
- Q2. Remind people to choose Kid’s Source code, so their responses will go under kid’s account
- Check how many people responded for you in the Dashboard below

FAQ
Step 4. If kids finish before day 20th of the month by Completing Form A + Collect at least 30 responses in Form B
- Check email for instruction to log in website: www.fundamentaldecisions.com
- On last day of the month: log in website & go to User profile/ View my report to see report
Note: If you don’t receive email with Log in instruction, go to website, Log in/ Forgot password. A recovery email will be sent to you to reset password & you can log in again.
Step 5: Parent/ kid follow instruction on Report to book a review call 1:1

- How to start doing recommended activities, submit exercises to get more comments & recommendation
Step 6: Parents and kids conduct the recommended activites & submit at least 1 result here for Johari Space Junior to:
- continuously reassess kid’s personality traits, thinking & emotional pattern
- analyse the change and what triggers the change
- provide more activities for deeper analysis & discover more of kids
